Application for a White Rose Charge-o-Plate. On this document there is a space for applicants to provide personal information such as their name, plate number, age, address, phone number, occupation, employer, address, references (bank, charge accounts, finance), marital status, provincial operators number, number of charge-o-plates required and the applicants signature. On the reverse side of the application, there is an explanation of why one should have a charge-o-plate which reads "for your convenience... to keep a record of automotive expenses and to reduce amount of cash carried on trips". Charge-o-plates where honoured in both Canada and the United States and were created by the Canadian Oil Companies Limited. |
